GRAFMONUMENT (vak 12, grafnr 125) bestaande uit een liggende zerk met licht hellende dekplaat die aansluit op een rechtopstaand gedeelte tegen de korte zijde. Dit eindstuk met afgeronde hoeken is bovenop voorzien van een soort kussen waaronder boven een bronzen medaillon een tekst, "Er trennt und er vereint". Het medaillon met vrouwenkop in profiel.
Liggende steen met tekst uitgevoerd in verheven letter:" Vrouwe Hendrika Lens-de Groot / geb. te Utrecht 5 jan. 1861 / gest. te Curacao 1 oct. 1908. Maurits Johannes Lens/ geb. te Paramaribo 17 juli 1891/ gest. te Utrecht 9 april 1898. Sofia Adriana Lens/ geb. te Curacao 10 oct. 1896/ gest. aldaar 20 oct. 1896.

GRAVE MONUMENT (plot 12, grave no. 125) consisting of a recumbent tombstone with a slightly sloping cover slab which connects to an upright section against the short side. This end piece with rounded corners has on top a sort of cushion beneath which above a bronze medallion a text, "Er trennt und er vereint". The medallion with female head in profile.
Recumbent stone with text executed in raised lettering: "Vrouwe Hendrika Lens-de Groot / born in Utrecht 5 Jan. 1861 / died in Curacao 1 Oct. 1908. Maurits Johannes Lens / born in Paramaribo 17 July 1891 / died in Utrecht 9 April 1898. Sofia Adriana Lens / born in Curacao 10 Oct. 1896 / died there 20 Oct. 1896.
