Les Templiers possédaient dès 1130 leur hôpital à Narbonne. Leur commanderie était d'abord située dans le bourg, île Sainte-Marguerite. Leur nouvelle chapelle fut construite en cité entre 1177 et 1208. Le nom d'Ile de la commanderie Saint-Jean est resté attaché à tout l'ilôt jusqu'en 1789. C'est dans cette chapelle que s'installèrent, en 1593, les Pénitents Bleus, ordre fondé en 1575. En 1619, ils obtinrent un bail avec faculté de bâtisse sur les vieux fondements. Par suite de désaccord avec le commandeur, ce n'est que dans le courant du 18e siècle que fut édifiée la chapelle actuelle. L'édifice est à nef unique, à trois travées, avec chevet polygonal. La construction du 18e siècle a tiré parti des murs déjà existants, surtout dans la partie nord, entre le chevet et la seconde travée. La façade occidentale a entièrement été édifiée à cette époque-là, en moellons taillés provenant pour une part du bâtiment antérieur. Le portail s'ouvre dans un avant-corps peu saillant, percé d'un grand oculus et surmonté d'un fronton triangulaire. Le long de la façade court un bandeau dont le profil de corniche romane rappelle peut-être le souvenir de quelque ancienne corniche. L'entablement, très simple, est surmonté d'un fronton rompu.
