L'hôtel-Dieu fut fondé en 1182, Nogent-le-Rotrou étant un lieu de passage sur le chemin vers Compostelle. Au 17e siècle, l'hôtel-Dieu était constitué par l'agglomération, autour de la chapelle, de petites maisons indépendantes et séparées. En 1641, Sully choisit de s'y faire inhumer, en dehors de l'église du fait de sa confession réformée. Sa veuve, Rachel de Cochefilet, fit élever un mausolée couvert d'ardoise et agrémenté d'une branche de lys, qui abrite le tombeau et les priants, sculptés par Baudin. En 1643 fut commandée la construction du portail permettant d'accéder au tombeau de Sully. De 1728 à 1732, construction de la salle des hommes à l'emplacement de la maison des Voûtes. A partir de 1772, la salle des femmes est élevée en contiguité. C'est dans ce bâtiment principal que fut placé l'oratoire particulier de l'hôtel-Dieu. Le bâtiment B sur cour était occupé par la pharmacie et le dortoir des soeurs de Saint-Vincent de Paul, qui dirigeaient l'hôtel-Dieu depuis 1672. Il fut modifié et agrandi en 1861. En 1818 est créé un hospice pour vieillards, dit "hôpital des Bonshommes" (emplacement du bâtiment C) , entièrement reconstruit en 1861. Les travaux se poursuivirent jusqu'en 1866, sous la direction de l'architecte Lebart, pour augmenter d'un étage le corps principal et édifier un avant-corps à l'est pour les salles de bain alimentées en eau courante. En 1868, élévation de la tourelle d'escalier sur la façade ouest. Un hôpital militaire est crée en 1878, en partie dans l'aile D et dans l'aile sur rue. L'hôpital s'agrandit encore en 1933 par la création de la maternité.
